The match days are 24th, 26th and 28th November 2021 at Nishigaoka in Tokyo.

y'-Teams shall be eligible to play in play-off match to decide entry into Japan Football League.

The ranking is determined as follows: 1) total of points, 2) goal difference, 3) total of goals.

When regulation time of the match play fails to determine a winner, the winning team is decided in a penalty shoot-out.

32 representive teams; Hokkaido 2 teams, Tohoku 2 teams, Kanto 7 teams, Hoku-shin-etsu 2 teams, Tokai 4 teams, Kansai 5 teams, Chugoku 3 teams, Shikoku 2 teams, Kyushu 4 teams and ONE host region (Tochigi) team.

The top three teams (A, B and C), except qualified team for the National Regional Soccer Champions League 2021, shall be eligible to play in National Regional Soccer Champions League 2021.
