In Takamado Trophy U-18 Football League 2011 Premier League, Twenty representatives(*) from nine regional Prince Leagues.
*) one team of Hokkaido, two teams of Tohoku, five teams of Kanto, one team of Hoku-shin-etsu, three teams of Tokai, two teams of Kansai, three teams of Chugoku, one team of Shikoku, two teams of Kyushu.
In the Premier League, all teams play each other two times (each at home and away).
Teams are allotted three points for a win, one point for a draw, and no points for a loss.
The ranking is determined as follows:) total of all points, 2) goal difference, 3) total of goals.

- 3: a Win
- 1: a Draw
- 0: a Loss
In the each Premier League, the top one team shall be eligible to play in the Takamado Trophy U-18 Football League 2011 Championship.
In the each Premier League, the bottom two teams shall be relegated to each regional Prince Leagues automatically.
In the each regional Prince League, the top one team shall be eligible to play in the play-off matches for Premier League 2012.
